Best way to make a pipe tee?

I'm trying to make a carbon steel pipe tee like this (minus the end chamfers): http://www.kinnaristeel.com/wp-content/uploads/2014/05/tee2.jpg

I've made them by solid operation thus far (merge pipe with a branch pipe to make the tee shape, make a slightly thinner one to subtract from the first shape) which usually ends up with missing faces when imported due to tangency issues between the main run of pipe and the branched pipe. I was told some time back the best way to make the shape would be from the constituent surfaces. A lot of ways come to mind as to how to make the surfaces, but has someone got a method that works reliably offhand? Any help would be appreciated, thanks.
